Metastatic carcinoma of the pituitary gland

A 71 year old woman presented with a 3 week history of weight loss, nausea, vomiting and severe depression. Physical examination disclosed no abnormalities. Her past medical and family history was unremarkable. Standard laboratory tests including electrolytes were normal. In order to detect the reason for her symptoms extensive diagnostic procedures (CT-Scans, endoscopy of the lower and upper GI-tract) were done, but no pathologic changes were found. Some weeks later she was admitted to the psychosomatic unit because of worsening depression and a drug therapy with mirtazepine was initiated. We were glad to see that so many of you joined our first ever CellQuiz competition at AACC. The contestants tested their morphology skills by matching cell class to the right cell image in just 60 sec. Congratulations to Alfonso Ziccardi, Assistant Manager AP/CP Hematology at the New York Hospital Queens, who scored highest and thereby won a brand new iPad.

The diagnosis

The patient developed Therapy Related MDS which later progressed to a case of AML where the patient had 60% blasts.

The images show the following:

  • Blast cells
  • Dysplastic granulocytes
  • Pseudo pelger huët
  • Peanut shaped cells
  • Dysplastic eosinophils with basophilic granules
