Cell Case #19

This Cell Case presents a 56 year old male. He came to the hospital with fatigue and dyspnoe d’effort complaints.

Result from the cellcounter:

A (macrocytic; MCV 117) anemia (7,3 g/dL) was detected and the cell counter flagged for an abnormal leukocyte population present. Leukocyte count was slightly elevated (15×109/L) with a mild thrombocytopenia (100 x109/L).

  1. These cells look like lymphoblasts. Not especially large, but with the fine chromatin in the nucleus and shape. It would be nice to see what, if any, normal leukocytes were present.

  2. Sure looks like an Acute Myelogenous Leukemia. These blasts are truly beautiful. I love the fine chromatin and large N:C ratio with basophilic cytoplasm. Anemia and thrombocytopenia are side effects of leukemia. I suspect the bone marrow would certainly be full of blasts with this peripheral picture. The blasts are not crowded on the slide because of the decreased RBC count, leaving more “white space” between cells. Thank you for posting this case.

  3. it seems to me that the four pictures are beautiful blast cells: acute myeloid leukemia (FAB classification M1) if all those white blood cells in the slide have the same appearance.
